We offer a monthly Telehealth Membership designed to provide convenient, accessible, and continuous medical care.
Membership Fee:
$100 per month
Membership includes:
Up to two (2) telehealth visits per month
Medical consultations via a secure virtual platform
Continuity of care with your provider
Terms & Conditions:
Membership is billed monthly and is non-transferable.
Payment must be made in advance to activate or maintain services.
Included visits are limited to telehealth services only and do not roll over if unused.
Additional visits beyond the two (2) included per month will incur additional charges.
This membership does not include in-person visits, laboratory testing, imaging studies, medications, procedures, or specialist referrals unless otherwise specified.
This program is not health insurance and does not replace comprehensive medical coverage.
Patients are responsible for maintaining active insurance coverage if desired.
Services are subject to medical appropriateness and provider discretion.
Cancellation Policy:
Membership may be canceled at any time with written notice at least 7 days prior to the next billing cycle.
No partial refunds will be issued for unused services during the billing period.
Failure to cancel within the required timeframe will result in the next monthly charge.

This membership is intended for non-emergency medical care only.
In case of a medical emergency, call 911 or go to the nearest emergency room.
The provider reserves the right to modify, suspend, or discontinue this program at any time with prior notice.
